Help?

I'm going to ask for an Incomplete grade (basically an extension) for one of my classes. Would you take a quick look at what I wrote and let me know if you think that I need to add anything?

I have fallen way behind in my assignments due to an illness. Specifically, I am being treated for clinical depression, and I can have a therapist or doctor confirm that if you tell me what to provide. I apologize for not saying anything earlier, but I kept on hoping that I'd bounce back quickly, and that hasn't been the case. I am hoping that you'll allow me to have a grade of Incomplete and make up the work. I could get everything done by the end of May.
