Adults responsible for your care actively helped facilitate your early learning. (Reading at bedtime, playing educational games, going to child-friendly museums...)
I taught myself to read at 1.5 years old. I learned it from TV commercials. Everyone bought me books (my family and family friends). (I'm still a bookaholic.) We also went to the museum of Science and Industry, Field Museum, Shedd Aquarium, and Adler Planetarium. I can't remember if we went to the Art Institute, but we probably did. And my dad worked for United Airlines and we traveled a lot.

You had a library card.
Yes. I took stacks of books home. I had to prove to one of the librarians once that I was really reading all those books.

Adults in your life involved you in tasks that involved mathematics skills.
My dad taught me arithmetic at a young age, including numbers in different bases.

If you started falling behind in school, you received help from a private tutor.
I didn't fall behind in school. Actually, I was a tutor in high school.

You went to a well-funded school.
Yes, Evanston Township High School.

If you were disabled and/or neurodivergent, you were classified by your school and received support through the education system.
I wasn't considered disabled or neurodivergent. That happened later.

You generally felt physically and emotionally safe at school.
No! I was bullied from Grades 1 through 8. One guy used to kick me in the stomach. The bullying left me with social anxiety that persists to this day.

You were in relatively good physical and mental health.
I had bad problems with Tonsillitis. My mental health was not great due to the bullying. Then in high school, I suffered a major depression.

For the most part, you were able to study and complete assignments without any struggle.
Yes.

Test-taking came easily to you.
Yes.

You seldom faced difficulty understanding assignments.
Yes.

You read at grade level or above.
I read at a high-school level at Grade 5.

Adults responsible for your care supported your academic journey for the better and for the worse.
Yeah, I would say so. I was able to choose my university, although I really wanted to go to New York University, and my parents wouldn't let me.
